Soup was cold, sandwich made wrong twice. Will never return

I like the restaurant and it's worth the trip (approx 20 miles). The food is usually good and the service is okay. However, on the last visit the bread was toasted and a little too hard, even though the food was still relatively tasty. It just wasn't as great of an experience as it usually is for us.
