I'm not sure what the intake/pre-screen form refers to. I've been a client for over 2 years, so perhaps this was something I filled out when I first began my membership.

It’s been a while since i got a massage and I didn’t have the greatest experience the last time I was at Hand & Stone, but Jami Pyle, my massage therapist was very good and the receptionist was super friendly. I also felt very safe with COVID and all.

The therapist Christopher was very intuitive, personable and managed all my problem areas perfectly. I left feeling relaxed and the tension was gone after my session. Also the receptionist was very professional and pleasant!

My massage therapist, Keegan Morris, is great - he really worked on the problem areas I identified. It's been a while since I've come for an appointment and he remembered me from last time. Personable, super friendly, and knowledgeable. 5/5 stars if there was a rating system here.
